A lumbar puncture was performed, but cultures were sterile. Analysis of the cerebrospinal fluid showed:
Blood tests showed a raised white cell count, elevated HbA1c and high ketone levels. Liver function test results were in the normal range.
Urine was strongly positive for glucose.
A CT scan showed a moderate sinus infection with mucosal thickening, resulting in obliteration of both sides of the ostiomeatal complex and the frontal recess.
MRI of Arjun's brain showed evidence of multiple strokes due to blockage of the right superior cerebellar artery, extensive sinus infection on his right side, widespread inflammatory changes in the deep spaces of the right head and neck and thrombosis in the right cavernous sinus.
White blood cells – 255 x 106/L
Neutrophils – 82 x 106/L
Lymphocytes – 18 x 106/L
Red blood cells – 10 x 106/L
Glucose – 156 mg/dL (CSF:blood ratio 0.78)
Protein – 6 mg/dL
